Documenting Acute Resuscitation Plans
Deciding on what to include in an Acute Resuscitation Plan (ARP) requires time talking to patients and carers about their wishes at the end of life. At Prince Charles improving the ARP documentation has reduced the number of Medical Emergency Team calls to dying patients. A parallel initiative to improve staff confidence to provide care for the dying is a documented plan of care that includes a list of clinical prompts, medication guidelines for the management of common symptoms and a restlessness and agitation flow chart. Auditing their performance ensures that there is a focus on continuous improvement.
