MeCare – Providing Specialised Support for People with Challenging Health Issues and Circumstances

At West Moreton HSS, Qld 5% of chronically ill patients consume 50% of the resources.

The chronic, high frequency and high-cost patients are given e-tools and protocol’s for monitored self-management at home.  All results are automatically analyzed at the MeCare Hub that is staffed by health professionals (allied health, psychologist, physicians, social workers, pharmacist, and nursing). 

The MeCare Hub has direct communications with patients, healthcare providers, and community support networks.  Patients become managers of their illness and if they deteriorate have a direct admission to HITH– bypassing ED and hospital.
